POLICY & PROCEDURE FOR NOTICE OF PRIVACY PRACTICES
The privacy practices of Homme, Inc. of WI, designed to protect the privacy, use and disclosure of Protected health Information (“PHI”), are clearly delineated in the Homme, Inc. of WI’s Notice of Privacy Practices (“Notice”) which was developed and is used in accordance with Federal requirements.
The privacy practices of Homme, Inc. of WI are described in the facility’s Notice.
The privacy practices and requirements of Homme, Inc. of WI are further detailed in the facility’s privacy policies and procedures.
The Notice is distributed to residents when Homme, Inc. of WI first provides care to residents upon request.
Homme, Inc. of WI makes a good faith effort to obtain written acknowledgement of a resident’s receipt of the Notice. When written acknowledgement cannot be obtained, there is documentation o explain the efforts made to obtain it and the reason(s) why it was not obtained.
The Notice is available to anyone who requests it.
The Notice will be revised as needed to reflect any changes in Homme, Inc. of WI’s privacy practices. Revisions to the Notice will not be implemented prior to the effective date of the revised Notice.
When revisions to the Notice are necessary, all current residents, employees, volunteers and business associates will receive a revised copy with notation of the changes made.
Homme, Inc. of WI retains copies of the original Notice and any subsequent revisions for a period of six (6) years from the date of its creation or when it was last in effect, whichever is later.
Documentation is retained for six years of a resident’s written acknowledgement of receipt of the Notice or of efforts made to obtain this written acknowledgement and the reason(s) why it was not obtained.
All employees, volunteers and business associates of Homme, Inc. of WI are required to adhere to the privacy practices as detailed in the Notice, privacy policies and procedures and Business Associate contracts.
Violations of Homme, Inc. of WI’s privacy practices will result in disciplinary action up to and including termination of employment or contracts.
The Notice is posted in a clear and prominent location.
The Notice is prominently displayed and available electronically on Homme, Inc. of WI’s Web site at http://www.homme.org.
