  • The Garden Apartments. What is an RCAC?

    The Gardens RCAC is a certified RCAC (Residential Care Apartment Complex) assisted living residence conveniently located on the campus of Forest Park Village, rich in history as it was formerly the St. Mary’s Hospital Campus. Here, you can continue living the life you enjoy, with an extra measure of assistance in daily living tasks. The Garden’s caring environment is designed to blend attentive personal care with respect for your privacy, safety and security.

    Licensed by the State of Wisconsin, we can provide up to 28 hours of care per resident. Care is determined through a health assessment and laid out in an individual service care plan to meet each person’s unique needs.

     

    What Do We Offer?

    • Twenty six private efficiency, one or two bedroom apartments
    • Appliances including refrigerator and stove; window blinds
    • All utilities including water/sewer, air conditioning and electricity/heat
    • 24 hour staff assistance
    • Personalized pendant emergency call system
    • Registered Nurse services and overall health monitoring
    • An individual care plan based upon an assessment conducted by the Registered Nurse
    • The noon meal and evening meals are included in the monthly rental fee
    • Weekly housekeeping and individual laundry services
    • A wide array of social and therapeutic activities and opportunities to socialize; many at no or low cost to you
    • Weekly shopping trips
    • Community social outings
    • Leases are on a month to month basis
    • On site Physical, Occupational and Speech therapy is available as ordered by a physician. Cost will depend on insurance coverage.
    • Regular health checks
    • Promoting independence while focusing on attention toward preserving dignity, privacy and freedom.

  •  Stone Crest Community Based Residential Facility. What is a CBRF?
  • Stone Crest Residence is a Community Based Residential Facility specializing in memory care for individuals with Alzheimer’s disease and other dementias. We offer specialized care in a beautiful environment that allows family members to know that their loved one is safe and secure. Residents receive 24-hour personal care, medication and incontinence management. Residents are protected by emergency response systems and supervised by our management and nursing team.

    What Do We Offer?

    • Sixteen private rooms with private half baths. Each room is equipped with emergency pull cord alarms.
    • We are a total care facility specializing in memory care. This includes but is not limited to dressing, bathing, peri-cares and more.
    • Under the State of Wisconsin’s definitions, we are class C; meaning we can accept ambulatory, semi-ambulatory or non-ambulatory residents.
    • 24 hour staff assistance
    • Registered Nurse services and overall health monitoring.
    • Medication management
    • An individual service care plan
    • All meals including snacks are included. Special dietary needs can be accommodated such as low sugar, low sodium or mechanically soft meals.
    • All rooms come with a bed, chair and night stand or bring in your own!!
    • A wide array of social and therapeutic activities and opportunities to socialize

    Who Can’t We Serve?

    • Anyone who is ordered bed-bound; unless enrolled in a hospice program.
    • Anyone who is physically violent as part of their disease process.
